Pantafon-DSR | Pantoprazole gastro-resistant and Domperidone prolonged release capsules IP

Product Details

Product monograph · updated 28 October 2026

Pantafon-DSR is Laafon Galaxy’s brand of pantoprazole and domperidone capsules: pantoprazole 40 mg in gastro-resistant (enteric-coated) form plus domperidone 30 mg, of which 10 mg is released immediately and 20 mg slowly. It is a Schedule H prescription medicine used for acid reflux (GERD) when a doctor also wants to speed up stomach emptying.

The detail most pages miss: one capsule already contains 30 mg of domperidone, the full daily ceiling the European Union set in 2014 because of heart-rhythm risk[1]. A second capsule in the same day would double it. The capsule is built for once-daily use, under a doctor’s direction.

What is inside a pantoprazole gastro-resistant and domperidone prolonged-release capsule

The label name, “Pantoprazole Gastro-resistant and Domperidone Prolonged-release Capsules IP”, describes three different release systems packed into one hard gelatin capsule. Each one exists for a reason.

Schematic only, to show the order of release. It is not a measured dissolution profile.

Why pantoprazole is “gastro-resistant”

Pantoprazole is broken down by stomach acid, so it is coated to pass through the stomach intact and release in the intestine. That is why it is a delayed-release medicine and should be swallowed whole, not chewed or crushed[2]. Earlier versions of this page described it as immediate-release (IR). That was incorrect: the Pantafon-DSR carton states EC (enteric-coated).

Why domperidone is split 10 + 20 mg

The 10 mg immediate-release part acts early. The 20 mg sustained-release part keeps a lower level going for longer, so one capsule can replace three separate 10 mg tablets. The total is still 30 mg a day, which is why the daily ceiling matters.

How pantoprazole and domperidone work together

Pantoprazole: cuts acid production

A proton pump inhibitor. It reduces the amount of acid the stomach makes[2]. In the US label, 40 mg once daily for up to 8 weeks is the adult dose for healing erosive oesophagitis caused by GERD, and 40 mg twice daily is used for Zollinger-Ellison syndrome[3].

Domperidone: moves the stomach along

A dopamine blocker that acts mainly outside the brain. It raises lower oesophageal sphincter pressure and increases stomach movement, so food and acid spend less time where they cause reflux[4]. It is also an anti-sickness medicine.

What the evidence shows

A 2022 meta-analysis pooled 11 randomised trials with 841 patients comparing a PPI plus domperidone against a PPI alone in GERD. The combination gave a greater reduction in overall GERD symptoms, and side effects were similar in both groups[4].

Practice in India is less tidy. A 2025 study of 8,839 outpatient prescriptions at tertiary hospitals found domperidone + PPI combinations in 481 of them. In 55% the dose was not written at all, and only 10.81% were for nausea and vomiting, the one use the EU still allows for domperidone[5]. The authors concluded that prescribing of these combinations was often not rational.

Pantafon-DSR dose: how to take pantoprazole and domperidone SR capsules

PointWhat appliesWhy
How manyOne capsule a day, or as your doctor prescribes. Never two in one day unless a doctor has deliberately chosen that. Each capsule is 30 mg domperidone. The EU limits adults to 10 mg up to three times a day, 30 mg in total[1].
WhenBefore a meal, usually before breakfast.Domperidone absorption is delayed if taken after food[6]. PPIs work best taken 30–60 minutes before a meal[5].
HowSwallow whole with water. Do not chew or crush. The enteric coat protects pantoprazole from stomach acid[2].
How longThe shortest course that controls symptoms. Review with your doctor if you need it for more than a week. The EU says domperidone should not normally be used for longer than one week[1]. Long-term PPI use carries its own risks (see side effects).
Missed doseSkip it and take the next one at the usual time. Do not double up.Domperidone label instruction[6].
ChildrenNot an adult capsule for children. Doctors use separate, weight-based products.Below 35 kg, domperidone is dosed at 0.25 mg per kg[1].

Before taking it: a 4-question safety check

Domperidone’s heart-rhythm risk is concentrated in specific groups. Answer these four questions to see whether any of the label’s red flags apply. It is a prompt for a conversation with your doctor or pharmacist, not a clearance.

1. Do you have a heart-rhythm problem, heart failure or a low potassium or magnesium level, or do you take a medicine that affects heart rhythm?

2. Are you taking itraconazole, ketoconazole or another oral azole antifungal, clarithromycin or erythromycin, or HIV protease inhibitors?

3. Do you have moderate or severe liver disease?

4. Are you over 60, or already taking another medicine that contains domperidone?

Answer all four questions to see the result.

Built from the contraindications and warnings in a current UK domperidone label (revised 31 March 2026)[6] and the EU’s 2014 restrictions[1].

Side effects and warnings

Pantoprazole

Common: headache, nausea, vomiting, diarrhoea, gas, joint pain and dizziness[2].

With long or high-dose use: the US label warns of Clostridioides difficile diarrhoea, bone fractures, vitamin B12 deficiency, low magnesium, fundic gland polyps, lupus-type skin reactions and acute kidney inflammation (tubulointerstitial nephritis)[3]. Fracture and polyp risks are highest at high doses or after a year or more of use[2].

Symptom relief on a PPI does not rule out stomach cancer, so persistent symptoms need investigation[3].

Domperidone

The serious one: QT prolongation, serious ventricular arrhythmia and sudden cardiac death. The risk is higher in people over 60, at more than 30 mg a day, and with QT-prolonging drugs or CYP3A4 inhibitors[6].

It also raises prolactin, so it must not be used with a prolactin-secreting pituitary tumour[6].

The US FDA has never approved domperidone for human use. In 2004 it warned of cardiac arrhythmia, cardiac arrest and sudden death when the drug was used to increase breast-milk supply[7].

Get medical help at once for a fast, irregular or pounding heartbeat, fainting, severe watery diarrhoea with fever, or a blistering or peeling rash[2].

Drug interactions to check before prescribing or dispensing

Two different mechanisms are at work. Domperidone is cleared by the liver enzyme CYP3A4, so CYP3A4 blockers raise its level[6]. Pantoprazole lowers stomach acid, which changes how some drugs are absorbed[3].

Medicine or groupClashes withLabel positionWhat happens
Itraconazole, ketoconazole and other systemic azole antifungalsDomperidone (and pantoprazole for ketoconazole)ContraindicatedPotent CYP3A4 inhibition raises domperidone levels. With ketoconazole 200 mg twice daily, QTc rose by a mean of 9.8 ms[6]. Lower acid also reduces ketoconazole absorption[3].
Clarithromycin, erythromycin, telithromycinDomperidoneContraindicatedCYP3A4 inhibition and added QT effect[6].
HIV protease inhibitorsDomperidoneContraindicatedPotent CYP3A4 inhibitors[6].
Other QT-prolonging medicinesDomperidoneContraindicatedAdditive QT prolongation. Apomorphine is the stated exception[6].

WarfarinPantoprazoleMonitorINR and prothrombin time can rise. Monitor INR[3].
Methotrexate (mainly high dose)PantoprazoleCautionMethotrexate levels may rise and stay high for longer[3].
Iron salts, erlotinib, dasatinib, nilotinib, mycophenolate mofetilPantoprazoleCautionThese need stomach acid to be absorbed, so absorption falls[3].

A combination to watch inside our own range: Itco-200 (itraconazole 200 mg capsules) is a systemic azole antifungal. It should not be taken alongside Pantafon-DSR or any other domperidone product.

Who should not take pantoprazole and domperidone capsules

  • Allergy to pantoprazole, any other “-prazole” PPI (substituted benzimidazoles), domperidone or any ingredient[3][6].
  • Known QT prolongation, significant electrolyte disturbance, or heart disease such as congestive heart failure[6].
  • Moderate or severe liver impairment[6].
  • Stomach or intestinal bleeding, blockage or perforation, where speeding up the gut could cause harm[6].
  • Prolactinoma (a prolactin-secreting pituitary tumour), or phaeochromocytoma[6].
  • Anyone taking the contraindicated medicines in the table above.

Pregnancy and breastfeeding: tell your doctor first. Domperidone passes into breast milk in small amounts[6]. It is not a milk-supply medicine; the FDA’s warning on that use is above[7].

Regulatory status of pantoprazole + domperidone in India (as of 28 October 2026)

QuestionPosition
Prescription statusBoth pantoprazole and domperidone are listed in Schedule H of the Drugs Rules, 1945[8]. That makes the capsule prescription-only, with the Rx symbol on the pack. See our Schedule H drug list (526 molecules).
Is the FDC approved?CDSCO approved several PPI + domperidone FDCs between 2004 and 2008. It later declined a dexlansoprazole + domperidone combination[5].
Banned FDC listsThe August 2024 prohibitions included sucralfate + domperidone combinations. The two-drug pantoprazole + domperidone FDC is not on that list. See 156 banned FDCs (2024).
Price control (DPCO 2013)The FDC has no ceiling price of its own. In the current NLEM-2022 ceiling list, S.O. 1575(E) of 25 March 2026, the scheduled forms are domperidone 10 mg tablet, domperidone 1 mg/mL oral liquid and pantoprazole 40 mg injection[9]. See the DPCO 2026 ceiling price list.
European UnionSince the 2014 Article 31 review, domperidone is only for nausea and vomiting, at 10 mg up to three times daily, normally for no more than a week. Oral products supplying 20 mg per dose were withdrawn[1]. A 30 mg sustained-release combination like this one would not fit EU rules.
United StatesDomperidone is not a legally marketed human drug. Access has been only through FDA expanded-access INDs, and the supplier for that programme stopped distributing after September 2025[7].

Indian schedules, FDC notifications and price orders are amended by gazette notification. Check the current CDSCO and NPPA position before relying on this table for a licensing or pricing decision.
